Program Information
Provides non-judgmental whole patient care for individuals affected by or suffering from substance use disorder. Provides in-house services and works closely with state and community stakeholders for warm-handoffs and referrals such as counseling, primary care needs, psychiatry, food, housing, employment and financial assistance.
Services can assist with various types of substance use disorders including, but not limited to, methamphetamine, opioids, alcohol and tobacco. Medication Assisted Treatment (MAT) utilizing addiction care specialists also available, as well as medication and pain management.
Individuals receive a dedicated care coordinator to develop customized plans with individuals to meet their needs and be an ongoing resource and support person for the individual as they navigate recovery.
