Program Information

Provides a variety of home health services.

Services include but are not limited to:

• Nursing
  • Nail care, diabetic testing and monitoring, injections, catheter and ostomy care, etc.
• Personal / Attendant Care
  • bathing, dressing, feeding, ambulation & mobility, incontinence care etc.
• Homemaking / Companion
  • Laundry, housekeeping, meal preparation, etc.
• Structured Family Caregiving
  • A program which provides a tax-free financial stipend to family members who provide personal care and support services to an individual in their private home or the private home of the principal caregiver.

Eligibility

Older adults, individuals with disabilities, and individuals with spinal cord or traumatic brain injuries
