Community Health Education / promotion / wellness and disease / accident prevention.

Provides transportation for medical purposes for individuals who qualify, transit tickets for medical appointments for able bodied individuals and Dial-A-Ride tickets, Community Health Worker services for high risk clients, and medication delivery for qualified individuals.

There are 3 CHR Specialists and 1 CHR Generalist that help with medical transportation and case management for high-risk populations.

Eligibility

Open to all Native Americans in the Pennington County area.

Disabilities access

Accessible parking, route, entrance, restrooms, or Braille signage.

Required Documents

Need a copy of Medicaid card for transportation billing.

Application Process

Call to set up appointment. Require 24-48 hour notice for transportation requests. Call or walk-in for a health risk screening.
