Patient Financial Services at Patient Financial Services at Sioux Falls - No Physical Address
Program Information
Patients can set up a payment plan by calling Patient Financial Services. Sanford Health also offers Financial Assistance for emergency and other medically necessary services provided and billed through centralized Patient Financial Services. Patients can call to request an application, or can visit the registration representative at the clinic or hospital and they will be able to provide an application.
The patient will need to return the application completed in its entirety along with:
- Copy of most recent complete tax return, including all schedules.
- Copies of the two most recent pay stubs for each wage earner in the household.
- Copies of property tax assessment statements
Applications will be reviewed and a decision will be communicated within 30 days. All information is confidential.
