Program Information
The transitional housing is part of the Community Support Services division of Southeastern. Psychiatric services to adults diagnosed with a severe mental illness who are in need of case management and/or medication management is provided.
The transitional housing is an 11 bed facility (each with their own private bathroom) that provides supportive services to individuals who are homeless or currently residing in an unstable environment who have a substance use disorder as well as a severe mental illness.
This is a sober living environment. Residents are provided 2 meals per day Monday - Friday and 3 meals Saturdays and Sundays. Residents are expected to participate in constructive daily activity outside of the transitional facility 9:00am - 3:00pm, Monday - Friday. Life skills groups and assistance with medication management are also provided. On site laundry.
Eligibility
Adults (18 or older) with a severe mental illness diagnosis and substance abuse disorder.
