The Care Coordination Program Manager will be passionate about health improvement and building community care coordination, referral, and data-sharing systems in collaboration with committed health care, human, and social service providers in South Dakota. The Care Coordination Program Manager will support the collaborative provider networks that support whole-person care using an interactive cloud-based platform that contains shared longitudinal records of each individual’s progress towards well-being. The successful applicant will have a balance of people and technology skills and have a track record that demonstrates their talents at engaging partners through group facilitation, individual conversations, excellent customer service, and technical support. This position would be ideal for a versatile and dedicated professional with supervisory skills and who can navigate multiple projects, relationships, and strategic priorities to create conditions for successful systems change.
Job Requirements
- Bachelor’s Degree with three years related experience and/or training; or equivalent combination of education and experience required.
- Preferred Master’s degree in Public Health or related human services experience
- 1-2 years of relevant professional and supervisory experience
- Project management experience desired
- Strong public speaking and communication skills
- Ability to influence others and move toward a common vision or goal
- Strong strategic awareness and innovative thinking skills
- Ability to read and interpret documents such as policy and procedure manuals.
- Ability to write routine reports and correspondence.
- Proficient in learning various technology software and integrations

Mental Health Counselors provide evidence informed crisis intervention, brief supportive counseling, suicide prevention, information & referral and follow-up services to individuals who contact the Helpline Center through 988 in emotional distress, with substance use issues or with suicidal ideation. Counselors interact with clients by telephone, SMS text messaging, and/or web chat. The position provides an opportunity to practice a wide range of clinical skills, and knowledge of the expanding field of behavioral health.
Job Requirements:
-Master’s Degree in counseling, social work, psychology, or related field required
-Additional experience working in mental health counseling or mental health crisis strongly preferred.

Overnight 988 Mental Health Counselor (Full-time, Sioux Falls)
This is an ideal position for individuals who are interested in helping others and making a contribution to their community. Schedule: 10:00 pm - 8:00 am Friday, Saturday, Sunday and Monday
*This role is eligible to be remote. Applicants must be located in South Dakota or a surrounding state.

The 988 Peer Support Specialist is responsible for providing non-clinical, strengths-based support to individuals experiencing or recovering from mental health and/or substance use problems.
Job Requirements:
- Bachelor’s degree in Social Services or related field.
- Demonstrated interest/skills in helping others achieve recovery goals.
- Minimum of two years in recovery from a lived experience with mental health and/or substance use.
- Strong oral and written English language skills.
- Working knowledge of computers and software programs, proficient keyboarding skills.
- Offers of employment contingent upon acceptable background check.
Provide support to individuals experiencing or recovering from mental health and/or substance use problems
- Provide non-judgmental, non-clinical, strengths-based support to individuals facing mental health and/or substance use challenges over the telephone.
- Be an active member of a multi-disciplinary team to provide support, coaching, and guidance.
- Connect individuals with community resources.
- Assist with developing recovery strategies and goals while maintaining healthy boundaries.
- Advocate on behalf of people with behavioral health problems.
- Coordinate warm transfers to 988 Mental Health Counselors for intervention or support when applicable.
- Participate in program evaluation to ensure projected goals are reached.
- Adhere to confidentiality and appropriate boundaries while providing peer support.
- Accurately and efficiently document client interactions.
- Assist with data collection as requested.
Hours: 8 to 12 hours/week. Hours are between 10am-10pm weekdays/weekends.
