Yankton Site
Provides skilled nursing facility with care with 24/7 nursing care. A comprehensive plan of care is developed and reviewed for each resident designed for individualized care with daily activities that address care for the whole person - physically socially, and spiritually. Special services include a Memory Care Unit and Adult Day services.

Wilmot Site
A 31-bed long-term care facility providing nursing and assisted living care. Also provides respite care, adult day services, hospice care, short-term rehabilitation, and therapy services.

Home Care Services: Provides services to help older adults or adults with disabilities stay in their home based on their needs. Services include home care and shore services, personal care services, adult companion, and nursing services.
Safety and Help Moving Around: Provides services in the home to help keep an aging adult or adult with a disability safe, secure, and independent. Services include medical equipment, medical supplies, and assistive devices.
Nutrition and Meals: Provides healthy meals, nutrition screenings, assessments, education, and counseling.
Senior Health Information and Insurance Education (SHIINE): Provides individuals information about the benefits they can receive through Medicare, and how to avoid fraud to protect their benefits.
Caregiver Support: Offers caregivers the resources and tools they need to care for their loved ones and gives them an opportunity to take a break from caregiving or receive a stipend for providing care to a family member. These services also provide aging adults and adults with disabilities time to socialize and receive support in a community setting through Adult Day Services.
Residential Living Services: Provides services and supports in assisted living centers, community living homes, and nursing facilities.
Ombudsman Program: Provides advocacy, education, and information for residents, family, and staff in long term care settings.
Adult Protective Services: Provides interventions to protect vulnerable adults from abuse, neglect, and exploitation.
